Responsibilities: Under the supervision of the Families Forward Crisis Services Coordinator, serves as a professional performing a wide range of direct and indirect mental health and substance abuse services within a multi-disciplinary, crisis-oriented team to provide immediate crisis intervention services to consumers with a chronic mental illness, possible developmental disability and/or co-occurring substance abuse issues. Provides in person and phone based crisis intervention and assistance with providing hospital referrals. Recommends referrals to various programs, program components and other services in the community. May provide case management; facilitation of a brief crisis intervention plan; outreach to consumers with multiple needs; and mental health services in a short-term crisis intervention program.
