JOB DESCRIPTION SUMMARY :

Under limited supervision, The Access Center RN is responsible for the management of patients who are pending admission to a gilchrist program. This position is responsible for following up on the patients whose admission is delayed and facilitating a timely resolution, so patients receive needed care sooner. This position maintains a data base of these patients which is used to trend and report the reasons for delays in care, suggest changes to workflow and/or program development opportunities to leadership regularly to allow for earlier care intervention for our patients and referrers. This position participates as a member of the Leadership Team and assists with the ongoing development of the Field Access RN’s and coordinators.
