**Job Summary and Qualifications**

The **Accountable Care Project Specialist** is responsible for leading initiatives that improve Memorial Health Partner’s (MHP) performance in its value-based care (VBC) programs and Medicare Accountable Care Organization (ACO), as well as support MHP’s direct-to-employer business. This position will oversee a portfolio of projects ranging from quality improvement, network analytics, practice transformation, payer contracting and network management.
