• Coordinate with patients and their attendants to schedule appointments for home health visits.
• Assist patients and families by providing relevant information, responding to queries, and addressing concerns.
• Arrange and manage home health visits as per the approved schedule.
• Liaise with doctors, nurses, and ward coordinators for home health referrals generated within the hospital for post-discharge services.
• Follow up with patients during their hospital stay and after discharge, and coordinate their home care management plan based on their specific needs.
• Maintain and update patient records and home visit data in a timely and accurate manner.
• Promote home health services within wards and clinics, and facilitate patient registration for home-based care.
