The Service Coordination and Management Division Referral Team (AD Disability Services) provides dedicated help to persons with disabilities and caregivers who require disability support and services, especially those not enrolled in any government-funded disability services. We enhance clients’ service journey through person‑centric support by assessing the needs of the families, coordinating case planning, providing information and advice as well as facilitating applications to suitable schemes or services in SG Enable or other agencies. The Senior Executive/Executive provides operational and administrative support to ensure smooth and efficient day‑to‑day operations.
