What will your responsibilities be?
Managing a caseload of adults requiring care and support, completing strengths‑based assessments, Mental Capacity Act assessments and developing person‑centred care plans.Working collaboratively with health professionals, community services and safeguarding partners to coordinate interventions, reduce risk and promote independence.Maintaining accurate case records, preparing professional reports and making evidence‑based recommendations for panels, best‑interest meetings and court where required.
