An exciting opportunity has arisen for an enthusiastic, motivated and committed Advanced Clinical Practitioner with strong clinical leadership and facilitation skills has arisen to join the Intensive Home Support Service (IHSS).

IHSS is an award-winning multidisciplinary team comprising ACP's, Nurses, Physiotherapists, Occupational Therapists, Nurse Associates/Assistant Practitioners and Medics. The primary aims of the service are to reduce the number of avoidable conveyances to hospital and provide a holistic and comprehensive assessment in the patients' usual place of residence. IHSS is commissioned to provide the 2-hour Urgent Community Response across East Lancashire and Blackburn with Darwen - which falls under the umbrella of Community Virtual Ward.
