• We are looking for an Advanced Nurse Practitioner(ANP) to join our team working 16 hours per week. Providing high-quality, patient-centred clinical care within a GPpractice setting. Working autonomously and as part of a multidisciplinary team,the ANP will assess, diagnose, treat, and manage patients presenting with bothacute and chronic conditions, ensuring safe, effective, and evidence-basedcare.

Main duties of the job

• Clinical Practice

Assess, diagnose, and treat patients presenting with acuteand chronic conditions

Undertake clinical examinations and interpret findings toformulate management plans

Prescribe medications in accordance with independentprescribing qualification (if applicable)

Provide advice on health promotion, disease prevention, andself-management

Manage patients with long-term conditions (e.g., diabetes,asthma, COPD)

Carry out home visits where appropriate
