Location: Bohemia, NY | Schedule: 35 hours per week | Status: Non-Exempt |Salary:24.73/Hour

The Opportunity


The Care Coordinator will explore Social Determinants of Health barriers to care and provide case management and health education for individuals and families receiving services in the Sherpa program. The Care Coordinator will communicate with outside providers and assist Sherpa's CRPA staff with management of referrals and complex care coordination as a means of increasing participant access to various resources and services.



Responsibilities


+ Complete Social Determinants of Health screenings and provide referrals to assist with barriers and priority needs

+ Monitor client progress towards improving access to care through follow-up with participant and service providers as needed.
