We are excited to be recruiting 4 Neighbourhood Care Coordinators. 

The Neighbourhood Care Coordinators will act as a trusted, named point of contact for residents with health, wellbeing and social care needs. Working across organisations within the neighbourhood partnerships, the post holder will support individuals to navigate services, overcome barriers, and access the right support at the right time.

This innovative role focuses on taking a whole-person approach to care, recognising that a resident’s health, social circumstances and personal goals are often interconnected. The Care Coordinators will work closely with health, social care and voluntary sector partners to ensure coordinated and person-centred support for people living in Buckinghamshire.
