ABOUT THE ROLE:

As a Care Coordinator, you will play an integral part as one of the key people liaising between our Clients, Support Workers and Package Managers. We are looking for someone who is confident, capable and ready for a challenge. Cover shifts, on-call.

You will need to demonstrate proven ability to coordinate people and systems and work as part of a team, supporting the Package Managers. You will be responsible for ensuring that all communication and documentation to assist our client’s rehabilitation is implemented.

MAIN DUTIES:

* Communicate weekly with available Support Workers (SW)

* Update the internal system with SWs availability

* Complete spot checks on staff to ensure safe and appropriate care delivery

* Carry out client visits as required and provide feedback

* To create rotas with support worker availability and confirm availability
