Care Coordinator – Youth Crisis Outreach Team (YCOT)

Role Responsibilities

Successful candidate will perform clinical screenings in schools or home settings for youth ages 3-17 experiencing a Mental Health crisis. Candidate must be highly competent in clinical triage, lethality assessment, de-escalation skills and engagement techniques. Candidate will use due diligence in determining the cause of each crisis and seek collateral information from others involved, i.e. hospital staff, law enforcement, school staff and family members. Candidate then consults with Texana psychiatrist or LPHA to obtain recommendations and helps facilitate that recommendation. Candidate will also coordinate necessary services including crisis follow-up and relapse prevention, referral and linkage to outpatient services.
