Description

The Care Navigation Coordinator works with children and families enrolled in program serving intellectually and developmentally disabled children ages 0-17. The Supports Coordinator is identified as the chairperson of the multi-disciplinary team and the developer of the Person-Centered Plan. They are responsible for assessment, referrals, linking and coordination of all services, as well as for monitoring and supervising prescribed services. They are also IDD Services Facilitator is also responsible for the clinical record.


Qualifications

EDUCATION AND / OR EXPERIENCE:

▪ Bachelor’s degree in psychology Social Work or related Human Services field required.

▪ Must meet criteria for QIDP credential and maintain that credential.
