About the Role

As a Care Partner, you will lead the delivery and continuous improvement of our Aged Care services across Townsville and surrounding areas. In this role, you will manage client referrals, support individuals as they commence services, and regularly review and update care plans to meet clients changing needs. You will coordinate home visits to ensure clients receive high-quality, person-centred care, while fostering strong relationships with clients, families, and key stakeholders.

You will contribute to the effective coordination of the program by reviewing, enhancing, and maintaining operational and administrative processes. With a strong focus on client experience, you will help ensure clients and their families feel supported and confident in the services they receive, while supporting the continued growth and success of our Aged Care programs.
