The Care Team Coordinator is dedicated to continuity of care, quality outcomes, and caregiver support by building trusted relationships, providing guidance and mentorship to care team members, and ensuring every client receives reliable, compassionate care.
Primary Responsibilities (including, but not limited to):
- Responsible for covering open shifts and/or last-minute callouts/no-shows during the work day.
- Introduces caregivers to clients.
- Conduct routine site checks of caregivers to ensure care plans are being implemented correctly, policies and procedures are adhered to, and all job duties are performed in a manner that is satisfactory and meets the Senior Helpers brand expectations.
Confirms that clients and their families are satisfied with the Senior Helpers services and communicates findings.
