  Client: 89-year-old male 


• 
  Condition: Fractured pelvis (post-rehab discharge) 


• 
  Living Situation: Lives with spouse and one small dog 


• 
  Start of Care: Saturday (discharge day) 

 
Care Needs
 

•  Hands-on assistance with mobility
  


• Bathing and personal care


• Housekeeping and laundry support


• 
  Transportation support (may require caregiver to drive client’s vehicle for errands/appointments) 


•  Companionship and in-home support
