The Centralized Intake Coordinator reports to the Manager of Centralized Intake and is responsible for ensuring all referrals are received, processed, and communicated promptly via fax, telephone, email, and electronic platforms. This role manages all aspects of referral acceptance and/or response, and coordinates closely with the local Gentiva family brand to ensure smooth operations and exceptional service to referral sources.



The schedules available:



+ **1 Part-Time Associate:** Saturday & Sunday, **8:00 AM – 8:30 PM**

+ **1 Part-Time Associate:** Monday, Tuesday, and Friday, **12:00 AM – 8:30 AM**
