About the Role
Mid Norfolk Primary Care Network (PCN) has an opportunity for an experienced clinical pharmacist, on a full time basis, to join their growing team, supporting six GP practices across the Dereham area.
The main responsibilities include:
Manage the prescription request workflow. Manage complex face to face medication reviews with an emphasis on seeing patients with polypharmacy, deprescribing, and opiate prescribing. Manage network quality activities such as sharing best practice and cross-network audit. Develop and manage medicines optimisation services within the network practices. Provide leadership on quality improvement and clinical audit as well as managing aspects of medicines safety and antibiotic stewardship. Support the management of patients with long-term conditions.
