Responsibilities

• Provide comprehensive medication management including reconciliation, dose optimization, and adherence support
• Partner with physicians and interdisciplinary teams to order individualized care plans under a CPA
• Monitor therapy effectiveness and safety to identify optimization opportunities
• Support quality-improvement and REMS initiatives
• Assist operations with complex prior authorizations and workflow improvements

Requirements

• Doctor of Pharmacy (PharmD) degree
• Licensed pharmacist in California
• PGY2 Ambulatory Care Residency OR PGY1 Residency with 1 year of ambulatory practice under a CPA OR 3 years of ambulatory practice with disease state management
• Proficiency in Microsoft Office tools and web-based platforms
