Job Summary:

Ensures appropriate, cost-effective drug utilization and pharmaceutical care while maintaining and enhancing patient care as an interdisciplinary team member in support of patients, providers, and ancillary staff. Provides members with continuous medication treatment where appropriate per authorization or utilizing authorized protocols supplied by software vendors built in the electronic medical record or authorized KPHI clinical pharmacy protocols.


Essential Responsibilities:


+ Provides pharmaceutical care services for defined high risk patient populations.

+ Ensures continuity of pharmaceutical care and optimal utilization of resources to and from acute and ambulatory patient-care settings.

+ Recommends and/or reviews laboratory tests to assess patient response to drug therapy.

+ Identifies medication therapy problems as appropriate.
