Summary The incumbent will function at the highest level of clinical practice working independently based on their scope of practice as a mid-level provider performing comprehensive pharmaceutical care. These activities include providing consultative/referral services for intensive medication therapy management (MTM) and disease state management. The incumbent will direct patient medication therapy by initiating, modifying and discontinuing medication therapy in an ambulatory care setting.
