Description and Responsibilities


Open Sky’s Community Compass Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.
The Community Compass Navigator will provide case management and direct outreach support to their assigned caseloads , which consist of members enrolled in Community Compass Program s , Recovery Support Navigation, Behavioral Health Justice Involved and/or C ommunity Support Program for People Experiencing Chronic Homelessness.
Other Key Responsibilities:
