Description and Responsibilities


Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.
The Program Manager operates within the philosophies of Harm Reduction, Motivational Interviewing and the Housing First Model to assist individuals in meeting their needs and foster empowerment for the persons served. They oversee the Community Supports navigation services, including hiring, supervising, mentoring, and evaluating navigators.
