What you’ll do
- Meet with clients to understand their needs, strengths, and goals.
- Create and deliver personalised support programs, including skill-building, social inclusion activities, and daily living assistance.
- Keep track of progress, write clear reports, and update plans as clients’ needs change.
- Link clients with other services and resources in the community.
