Responsibilities

• Perform various key functions within the Integrated Northern Health portfolio.
• Lead and sustain improvements in purposeful rounding, restraint management, falls prevention, injury reduction and other Continuing Care (CC) priorities.
• Work collaboratively with the interdisciplinary team to ensure best leading evidence-based practices are used to provide high quality, safe care.
• Provide on-site education to nursing teams to build capacity and create a positive, supportive care environment for residents and staff.
• Collaborate on development of policy and procedure for CC.
• Seek out best, leading and evidence-based practices for implementation within the care team.
• Role model culture change and resident-directed care within CC.
