Job overview

This is a senior role within the Integrated Discharge Team based primarily at Basingstoke but with the requirement to travel to Andover and Basingstoke in order to provide adequate nursing cover.

The team works with the multi-disciplinary teams within HHFT as well as with partners from the community to identify appropriate ongoing care for some of our most vulnerable adult patients and to manage the processes which expedite safe and efficient person centred discharge. The person will have knowledge and understanding of discharge pathways with a real interest and ability to facilitate conversations with system partners, wards, clinicians about appropriate onward care settings. They must be a skilled communicator with the ability to explain complex arrangements to patients and families as well as perform a teaching role on the wards.
