Performs patient assessment and plans, delivers and evaluates patient education to selected populations across the continuums of illness and lifespan. Assumes a role as a resource person by participating in community outreach and education support.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC). 

2. Obtain Certified Diabetes Care and Education Specialist (CDCES) Certification through theCertificationBoard for Diabetes Care and Educationwithin two (2) years ofhire date. 

3. Obtain certification in Basic Life Support within 30 days of hire date.

EXPERIENCE:

1. Two (2) years of clinical experience with One (1) year in the care of diabetic patients.
