**Job Description :**

**GENERAL OVERVIEW:**

Provides specialized clinical nutrition assessment. Provides guidance and support to dietitian team to include dietetic interns where applicable. Serves as lead for nutrition initiatives.

**ESSENTIAL RESPONSIBILITIES**

+ Performs individualized patient assessments and reassessments for most complex and/or high risk patients.
+ Develops, documents, and monitors the nutritional plan of care for the population served according to individualized assessment and professional standards.
+ Performs documentation and completes reports according to procedures and established practice and guidelines.
+ Provides education and/or educational programs for patients and staff.
+ Develops, implements and presents a minimum of one educational project annually.
+ Develops and leads a minimum of one nutrition initiative annually.
