Job Summary

Under the direction of the Director of Case Management, the Discharge Planner is responsible for establishing an appropriate discharge plan for patient’s in the acute care hospital setting.


Core Job Responsibilities


+ Responsible to perform psychosocial assessments to determine the patient’s discharge planning needs

+ Participate in progression rounds to report anticipated discharge planning barriers

+ Act as an advocate for patients and families to ensure that they are aware of resources available to them in the community

+ Collaborate with the CM/Patient Type Status Nurses to ensure that the patients are in the correct level of care

+ Responsible to establish an appropriate discharge plan and arrange services to meet the needs of the patient
