As a Discharge Planning Assistant,  you’ll   assist   with  facilitating   successful discharge plans which are critical to length of stay reduction and patient flow.   

Join a team of Care Management, Social Work, and Nursing, in providing  a high level   of customer service to patients by ensuring a proper and  timely   discharge. Make regular rounds of all patients to  assure   that the thoughtfully crafted treatment plans are being implemented and communicated across the interdisciplinary team.
