Main duties of the job The successful candidate will: work with care co-ordinators and 35 - 40 service users, depending on complexity and other responsibilities
carry out drug and alcohol misuse assessment including assessment of personal strengths and recovery capital, and risks
develop recovery plans using a person-centred approach to develop, implement and evaluate needs and strengths-based care plans and risk management plans in partnership with service users, carers and other professionals
