Developmental Services: Transitional Aged Youth Home – Toronto, ON
Full‑time, Permanent – Union position – $61,645.39 per annum

Skilled and compassionate caseload managers who work with developmental and dual‑diagnosis youth to support their transition from the child welfare system and help them build life‑skills for independent living.

Responsibilities

• Needs Analysis
  • Collaborate with individuals to develop goals and strategies that align with their unique needs.
  • Conduct effective needs analysis to identify individual strengths and gaps.
• Service Delivery
  • Support the delivery of services and supports according to goal plans.
  • Locate programs, services, housing and employment opportunities to meet goal plans.
