**Job Summary and Responsibilities**

As our part time **Care Coordination Licensed Masters Social Worker,** you will be instrumental in providing comprehensive social work assessments and interventions for emergency department patients. This includes crisis intervention, high-risk screening, and patient/family support to facilitate safe discharge planning and ensure continuity of care. You will be a key advocate for patients and families, navigating complex healthcare needs, and connecting them with vital community resources.

Every day you will perform psychosocial assessments, provide counseling support, and identify needs related to abuse, neglect, trafficking, and social determinants of health.
