The organization's mission is to help seniors remain safely in their homes and communities for as long as possible by offering coordinated care that includes primary and specialty medical services, rehabilitation, social programs, meals, and transportation.

About the Opportunity:

• This position will focus on care in our skilled nursing facilities
• Monday through Friday, 8 am-4 pm
• Average daily patient census is 8-15
