You will contribute to the success of DPV Health by operating as a Case Manager within the Healthy Mothers Healthy Babies team.

Working as part of the Healthy Mothers Healthy Babies Team, and reporting to the Team Leader- Outreach, you will be responsible for the provision of a high standard of service to eligible clients in the community.

This will involve you supporting self-management approaches via assessment, care coordination and education to clients with a range of presentations during pregnancy and in the immediate post birth period.
