Job Summary:

The mission of SUNY Upstate Medical University is to improve the health of the communities we serve through education, biomedical research, and health care. Beyond our downtown campus, we provide care through our clinics and offices across the greater Syracuse area. 

In support of our ongoing growth, we are seeking highly skilled and motivated Advanced Practice Providers to join our Heart Failure Clinic Team. The fulltime Heart Failure APP will provide care to patients in the outpatient setting at SUNY Upstate University Health Care Center (UHCC).
