CLCH Merton Adult Community Neuro Rehabilitation Team - Lead Occupational Therapist. We are seeking an experienced Specialist Occupational Therapist with expertise in stroke and neurological rehabilitation to join our Integrated Community Neuro Rehabilitation Team in Merton. Our service follows the national model for community neuro rehabilitation and provides person-centred care across Early Supported Discharge (ESD), Community Stroke Rehabilitation and Community Neuro Rehabilitation pathways. Working within a multidisciplinary team, you will support people recovering from stroke and those living with a range of neurological conditions, including Motor Neurone Disease (MND) and Parkinson's Disease (PD).
