UPMC at Home is hiring a Home Care Aide to support home health and hospice patients in Venango County and portions of neighboring counties. This position is a Monday-Friday, 8:00A-4:30P position with occasional weekend shifts for hospice patients only.

Responsibilities:

+ Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
+ Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
+ Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
+ Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
