The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.

+ 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays

This position is on site in our Farmingdale office located at 110 Bi-County Boulevard

Job Details

Our Home Care Transition Nurses:

+ Conduct patient assessments and develop individualized transition plans

+ Coordinate referrals and facilitate smooth transitions to home care services

+ Build and maintain strong relationships with physicians, discharge planners, and referral sources
