Salary Range: $18.65 To $22.00 Hourly







This position will be doing home care visits to our western Monroe county region (Brockport, Holley, Clarkson, Bergen, Hamlin, Spencerport, Hilton Kendall etc.)



Role and Responsibilities


Provide personal care and supportive services to individuals in the community, in the privacy of their home.





Essential Functions


+ Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.

+ Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
