**Schedule:** 40 hours guaranteed, 8:00AM – 4:30 (Monday – Friday)

**Coverage Areas:** East Baton Rouge/ Livingston Parish

· RN case manager for a specific territory

· Performing skilled routine visits, recerts, discharges and start of cares/resumption of cares.

· Each case manager is assigned their own territory but cover each other on days off

· 100% patient care

· OASIS visits, wound care, picc line care
