**Type:** Two-month contract

**Schedule:** 40 hours, 8:00AM – 4:30 (Monday – Friday) with on call

**Coverage Areas:** Newburgh NY territory

**Primary Responsibilities:**

· RN case manager for a specific territory

· Performing skilled routine visits, recerts, discharges and start of cares/resumption of cares.

· Each case manager is assigned their own territory but cover each other on days off

· 100% patient care

· OASIS visits, wound care, picc line care
