Position Overview

The Home Health Clinical RN is responsible for delivering skilled nursing care to patients in their homes, with a focus on OASIS assessments, care plan development, and ongoing return visits. This role supports patient recovery and independence through comprehensive clinical care, education, and coordination across disciplines.

Schedule & Work Details

• Coverage Area: Clearfield and Centre County, PA


• Schedule: Monday through Friday, 8:00 AM to 4:30 PM


• Weekly Hour Guarantee: 36 hours


• On Call/Weekend Requirement: Yes


• Mileage Reimbursement: Yes, reimbursed between visits from first patient to last patient


• Pay Rate: $51 to $53 per hour
