Description for Clinical Case Manager for Home Health Office

Our In-Office Case Manager primary responsibilities include but are not limited to:


+ Review of RN's OASIS Assessments to Ensure Completion and Accuracy

+ Record Case Information and Complete All Necessary Forms with Accuracy

+ Assist with Case Management including Draft Service Plans, Review Case Progress, Determine Case Closure

+ Facilitate Multiple care aspects including Case Coordination, Information Sharing, Discharge

+ Ensure Requests for Visits are Filled Out and Sent to Appropriate Staff

+ Follow-up on Patient Cases

+ Oversee All Patients to Ensure Care is Initiated in an Efficient and Timely Manner
