**Pay:** $51.00 - $55.75 per hour based on experience

**Type:** Two-month contract

**Schedule:** 8:00AM – 4:30 (Monday – Friday)

**On Call Requirement:** every other weekend on call - 3 nights per week

**Coverage Areas:** Luzerne and Lackawanna county

**Primary Responsibilities:**

· Admissions, discharges

· Recertifications

· Comprehensive patient care

· Care plan review

· Medication monitoring and ordering

· DME ordering, supply ordering

· Death visits

· Personal care
