Seeking a dedicated and detail-oriented Case Manager to join our Housing team. Manages a primary caseload of chronically homeless individuals eligible for rental assistance with weekly to biweekly contacts by home visits and telephone. Identify unmet service needs that may impact housing stability and provide clients with access to support services and referrals for successful housing stabilization. This role reports to the Program Supervisor and ensures compliance with program requirements Duties and Responsibilities Meet individually with clients for the purpose of monitoring clients' well-being and making referrals for supportive services at other agencies.
