Job Description
Primarily assigned to the Emergency Department, the Medication History Technician is tasked with reviewing and updating patients’ medication lists within the electronic medical record to ensure the utmost accuracy for individuals being admitted to the hospital. This process entails a thorough review of office visit documentation, recent hospital discharge summaries, and pharmacy records, in addition to conducting interviews with patients and their family members to confirm all current medications.
The technician is responsible for verifying the names, dosages, and administration instructions of all medications, including prescription drugs, over-the-counter supplements, sample medications provided by physicians, and medications taken on an as-needed basis. When discrepancies arise or information is incomplete, the technician may contact pharmacies or healthcare providers to obtain clarification or additional details.
