Provides individualized medical nutrition therapy to ambulatory and hospitalized patients as a part of a multidisciplinary team in a tertiary care center. Serves as a consultant on nutrition care for other members of the health care team. Applies evidenced-based medical nutrition therapy standards of care to incorporate the nutrition care process: nutrition assessment/re-assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring/evaluation. Completes thorough assessment, including nutrition focused physical exams, review of medical record for anthropometrics, medical history, labs, food allergies/intolerances, and drug-nutrient interactions. Identifies diagnostic criteria, including malnutrition. Interventions include developing advanced/specialized nutrition plans and writing orders/recommendations for therapeutic diets, oral nutrition supplementation, vitamin and mineral repletion, and nutrition support (enteral and parenteral) for high acuity/medically comp...
