            Job Description Physician needed in Suburban Jacksonville,Florida 
Convenient location minutes from Atlantic Ocean
100% Geriatric patient population
Open to experienced providers
Internal Medicine - Family Medicine - Geriatrics
Outpatient ONLY - Medicare Advantage patients
Average15-17 patients per day with full support staff
Physicians drive the care decisions for the patients
Compensation and BenefitsNegotiable $260,000 base salary
Up to 20% of Base Salary Bonus (average bonus is $50,0000)
Sign-On Bonus $35,000+
Relocation assistance if over 50 miles
Over 32 total days off including Holidays-Personal-PTO
