Responsibilities:
- Provide primary care services for an adult patient population age 55 and older
- Manage an average daily patient volume of approximately 20 to 22 patients
- Diagnose and treat acute and chronic conditions within the scope of family medicine, internal medicine, or geriatrics
- Work collaboratively with the site medical director and care team
- Deliver patient-centered care in a high-touch outpatient environment
